Valley Health Partners Community Health Center (VHP) is a new non-profit organization dedicated to advancing the health & wellness of the Lehigh Valley vulnerable community members and recognized as a Federally Qualified Health Center-Look Alike (FQHC-LA). This will be a new site providing ophthalmology and optometry services. Ophthalmology will be offered by a contracted Ophthalmologist from Lehigh Valley Health Network (LVHN). The site will also have an embedded Case Manager to provide care coordination for the vulnerable community members serviced at VHP.
